Working Together to Build Healthier Communities
Ontario Health Teams (OHTs) organize and deliver care that is more connected to people and providers in their communities. OHTs aim to strengthen services locally and to make it easier for patients to get the care they need, when and where they need it. There are 58 OHTs across the province with the goal to ensure that everyone in Ontario can benefit from better coordinated, more integrated care. Our Strategic Plan
The Ottawa West Four Rivers Ontario Health Team Strategic Plan 2024–2027 outlines the shared vision and priorities guiding the organization’s work to improve health and well-being across the region. The plan focuses on building a simplified and coordinated system of care by helping people connect to services, strengthening primary care, improving health equity, and supporting healthier communities through prevention and population health approaches.
It highlights collaboration with community partners, Indigenous communities, health providers, and neighbouring Ontario Health Teams to address local needs, enhance patient experience, and improve access to integrated care.

Meet Your OHT Team

Nadia Prescott
Executive Transformation Lead
Nadia provides the overall strategic, operational and administrative leadership, overseeing and supporting all aspects of the development, implementation and growth of the OHT. Nadia has a Master’s in Health Administration and has held several project management roles leading to the successful deployment of many strategic and Connected Care projects. Nadia previously worked at the Champlain LHIN as an ED/ACL Coordinator and Senior Integration and Accountability Specialist, where she got to know many service providers in our region and learned about the services they provide.

Meghann Darroch
Engagement and Capacity Building Lead
Meghann advances meaningful lived experience engagement, equity-driven collaboration, and capacity building across a diverse network of healthcare, community support, and social service partners serving urban, suburban, rural, and small-town communities. Over the past 25 years, she has held leadership roles in mental health, addictions, substance use health, and regional systems planning, including positions with the Canadian Liver Foundation as Regional Manager Eastern Ontario, CAMH as Regional Implementation Coordinator, and United Way as Manager of Capacity Building. She holds a BA in Sociology and Criminal Justice from the University of Guelph. Known for her relational leadership style, Meghann is grounded in connection, reflection, and a deep respect for lived experience.

Project Manager – Digital Health

Raine Hindley
Performance Measures Lead
Prior to joining the OHT, Raine held Data Verification roles with Ottawa Public Health and served as Acting Team Leader with the Immunization Team. Raine holds a Bachelor of Economics from Carleton University. While working with the OHT, Raine has also pursued additional education in project management, including successfully completing the Project Management Institute’s Certified Associate in Project Management (CAPM) certification in June 2025, and is currently working toward obtaining the Project Management Professional (PMP) designation. Since joining the OHT, Raine has supported a range of initiatives focused on advancing data-informed decision-making and system performance.

Dr. James Fullerton

Dr. Colby Burns
Dr. Colby Burns has been practicing family medicine for 13 years. He also is a part of the Rideau Palliative Group. He has been one of the Primary Care Council Digital Leads for the last year.

Chris LeBouthillier
Chris LeBouthillier has been working in primary care leadership for 15 years in Toronto and now as the executive director of the West Carleton Family Health Team. As one of Ontario’s first Physician Assistants, he previously practiced in a busy primary care setting and brings firsthand experience with many of the operational and clinical challenges facing primary care today. Chris also serves as the co-chair for the Digital Health, Privacy and Information Sharing Committee and the Primary Care Council.

Dr. Karen Ferguson
Dr. Karen Ferguson has been practicing family medicine for 27 years. She is the current FHO co-lead for the West Carleton Family Health Team. She has been the Primary Care Clinical Lead for the last year.

Dr. Aqeel Degani
Dr. Aqeel Degani has been practicing family medicine for 16 years. He is the FHO lead for the Rideau Valley Health Services. He has been one of the Primary Care Council Digital Leads for the last year.
