What is a Primary Care Network?
Primary Care Networks (PCNs) are the mechanism through which primary care clinicians within an OHT connect, coordinate and act collectively to plan, support, and implement provincial and local priorities.
Our PCN continues to evolve and strengthen through the active participation of family physicians, nurse practitioners, physician assistants, and other specialists. Collectively, more than 375 clinicians actively practice within our PCN and the communities we serve.

The work of our PCN is done in alignment wit the vision and guidance provided by the Ministry of Health and supports the Ontario’s Primary Care Action Plan to connect every person in Ontario to primary care by 2029.
Working Together, With You
A Primary Care Council (PCC), representative of our broader PCN, has been established as the leadership structure of the broader PCN. The PCC provides leadership, expertise, and decision-making support to the Ottawa West Four Rivers OHT regarding issues relevant to primary care clinicians and their patients. Lived experience partners are also represented within the PCC.
