Join the Long-Term Care Community of Practice for the second session in our series focused on Frailty in Long-Term Care. This session will explore the importance of recognizing and understanding frailty among residents living in long-term care and how this knowledge can support informed, compassionate care planning.
By the end of the session, participants will be able to:
· Develop a clearer understanding of frailty and the role of prognostication in long-term care.
· Recognize the legislative landscape and explore when a palliative approach may be appropriate for residents experiencing frailty.
· Understand the role of frailty indices and how they can help predict mortality risk and support clinical decision-making for residents in long-term care.
