Integrated Care Action Summit

The Integrated Care Action Summit (ICAS) is Ontario’s only province-wide event by Ontario Health Teams (OHTs) and Primary Care Networks (PCNs), for OHTs, PCNs, and their partners. It brings together all 58 OHTs, PCNs, the Patient, Family, and Caregiver Advisory Council, and partners from across the continuum of care to strengthen connections, exchange learnings, and advance integrated care across Ontario.
Building on the momentum of the 2025 Summit, this year’s event will focus on translating collective insights into practical and measurable actions that can be sustained across the province.
